Coin of Life and Death

Wondrous item, Legendary

One side of this shimmering coin represents Life as a beautiful warrioress. The other side is detailed with a vision of Death as a knight.

As an action, you can choose to flip this coin and utter the words "Amor Fati". If the coin lands on the symbol for "Life", you will be instantly resurrected the next time you fail three death saving throws or suffer the effects of instant death, such as from a disintegration spell. If the coin lands on the symbol for "Death", an avatar of death appears to claim your soul. 

It appears in a space of the DM's choice within 10 feet of you and attacks you, warning all others that you must win the battle alone. The avatar fights until you die or it drops to 0 hit points, whereupon it disappears. If anyone tries to help you, the helper summons its own avatar of death. A creature slain by an avatar of death can't be restored to life.

If you have not flipped the coin and drop to 0 hit points, the coin will fall from your person. If the coin lands on the symbol for "Life", you immediately succeed a death saving throw. If the coin lands on the symbol for "Death", you immediately fail a death saving throw.
